In order to put your child on the waiting list to the Nursery, please fill in the following form. We will contact you as soon as a vacancy occurs.

Signing your child up for the list gives the parents priority to the vacancy occurring, it is not however a guarantee of enrolment of the child to the Nursery in the requested time.

I agree for my personal data and my child personal data to be processed in order to put my child on a waiting list to the Nursery according to the Polish Law. I give the personal data by my own will and am aware that it is necessary for the enrolment process. I was informed that I have the right to check, correct and withdraw back the data.
